HSO card - Currently high risk, but HSO card is missing (Primary User)

1. Go to the 'MySJ Trace' page 

2. Click the 'Help' button on the top right of your 'COVID-19 Risk Status' widget 

3. Choose '2 My HSO card is missing or is not displayed on my screen'
